Terms of Engagement
This document outlines the framework for the nutritional therapy services offered by Caroline Sheldon (“the Registered Nutritional Therapy Practitioner” (RNTP)) of Midlife Management.
You will discuss and agree what support you want from the RNTP in your Foundation Session and review the progress and focus of your work together at subsequent consultations.
The RNTP uses the Wellsum client management system to provide a secure and efficient means of issuing documents to and receiving documents from clients. Please do not put any confidential information in an email. Please use the Client Portal to upload any test results and confidential material which you wish the RNTP to consider. Please book a follow up call if you have any specific questions for the RNTP concerning your health protocol.
The client should use the Client Portal to book and pay for consultations, complete intake forms, review their health protocol and test results and to upload any blood tests, clinical letters and any other material relevant to your health and well-being.
Please note that the cost of any supplements or testing recommended by the RNTP are not included in the consultation fee.
Payment
When the Foundation Session is booked, an invoice will be issued via the Client Portal. Payments will be made online using Stripe. Payment of the invoice confirms each individual consultation and must be made at least 48 hours prior to commencement of the consultation.
You are required to pay for consultations which are missed or cancelled with less than 24 hours’ notice.
It is confidential that you are a client of the RNTP. The content of your consultations with the RNTP is also confidential. However, there are some limits to duty of confidentiality which include if the RNTP has concerns about your safety or the safety of another person. You will be asked to provide details of your G.P. and your G.P. may be contacted in such circumstances. For further details, please see the GDPR Agreement. The RNTP will keep a record of your name, contact details and dates and times of consultations using the Wellsum client management system. The RNTP is required to keep notes as to the focus of your work.

The degree of benefit obtainable from Nutritional Therapy may vary between clients with similar health problems and following a similar Nutritional Therapy programme.
Nutrition and lifestyle recommendations will be tailored to support health conditions and/or health concerns identified and agreed between both parties.
RNTPs are not permitted to diagnose, or claim to treat, medical conditions.
RNTP recommendations are not a substitute for professional medical advice and/or treatment.
You are obliged to inform your RNTP about any changes which may occur to your medical records, medication and health.
The nutrition and lifestyle recommendations are intended to encompass your requirements as much as possible. Whilst every effort is made to ensure that the recommendations are both safe and effective, there is a risk of adverse outcomes from a nutritional protocol and you acknowledge and agree that you are taking part in the nutritional protocol of your own free will and are aware of the risks as stated.
Your RNTP may recommend food supplements and/or functional testing as part of your Nutritional Therapy programme and may receive a commission on these products or services.
Any changes in your diet and lifestyle may produce effects in your body, energy, health and condition that are gradual in nature, not instantaneous.
No advice or recommendations shall be applicable beyond 3 months following your final consultation. If you choose to continue with the advice or recommendations, this is done at your own risk.
Standards of professional practice in Nutritional Therapy are governed by the Complementary and Natural Healthcare Council (CNHC) Code of Conduct, Ethics and Performance.
This document only covers the practice of Nutritional Therapy within this consultation, and your practitioner will make it clear if she intends to provide services that are outside the scope of nutritional therapy practice.

The Client understands and agrees to the following -
I am responsible for contacting my GP about any health concerns you may have.
I give permission for my RNTP to contact my GP regarding any agreed aspects of my case.
If I am receiving treatment from my GP, or any other medical provider, I should tell him/her about any nutritional recommendations provided by my RNTP. This is necessary because of any possible reaction between medication and the nutritional programme.
It is important that I tell my RNTP about any medical diagnosis, medication, herbal medicine or food supplements that I am taking, as this may affect the nutritional programme.
I must tell my RNTP promptly if any of the new changes to diet or lifestyle cause adverse effects.
If I am unclear about the agreed nutritional therapy programme/food supplement doses/time period, I should contact my RNTP promptly for clarification.
I understand that the recommendations are personal to me and may not be appropriate for others.
I must contact my RNTP should I wish to continue any specified supplement programme for longer than the original agreed period, in order to avoid any potential adverse reactions.
Recording consultations using any form of electronic media is not allowed without the written permission of both me and my RNTP.
It is my responsibility to ensure that I have a working internet connection for online appointments. If I am unable to connect as scheduled, the appointment is considered a 'no show' and will be charged in full.
